Support Assistants
Our Enso Support Assistants can help participants gain independence and build relationships with our focus on participant well-being, health, and happiness. Whether it’s connecting with your community, getting some help around the house, or joining centre-based skills development and social programs, we strive to empower participants.

Allied Health Assistants
As an Allied Health Assistant, therapy support will be provided under the direction of your qualified Allied Health Professional (AHP). The allied health assistant will follow AHP-prescribed therapy and then provide progress notes directly to the appointed AHP.

GST
GST will not be charged on supports as advised below schedules 1 and 2

As an unregistered NDIA provider, we are not obligated to use the NDIA pricing limits however, we do comply with the current NDIA pricing limits as set out in the guide and charge participants according to the current NDIS price guide for PLAN-managed and SELF-managed participants. We require the participant's NDIS reference code for billing purposes. Additional fees apply for travel and are charged as per the NDIS price guide. Upon signing this document, you agree and understand that fees are subject to change in accordance with NDIS price limits and FEE CHANGES WILL TAKE AFFECT IMMEDIATELY ONCE NDIA ADVISE OF UPDATED FEES. 

Travel Additions
Participants will incur travel time for support worker time travelling to and from appointments. Participants will also incur car costs for transportation under Provider travel-non-labour for car costs at $1 per kilometre.

* Written consent from the school/daycare will need to be obtained by the parent/guardian and returned before visits can occur.

All appointments are billed by the hour and are broken down as follows:

45-minute face to face session + 15 minutes for preparation, planning, and notes

FEES PAYABLE
We ask that payment be made the same day as the invoice through direct funds transfer and no later than within a week of the service date. If payment is not paid within the week time frame and we are required to follow up with you, please note that .25 time will be added to the following invoice. In the event an invoice is refused due to lack of NDIS funding, you agree that the participant/parent/guardian will be responsible for paying outstanding invoices.

CANCELLING APPOINTMENTS
It is appreciated if you can give as much notice as possible if you need to cancel an appointment. Where a provider has a Short Notice Cancellation (or no-show), we are able to claim 100% of the
the agreed fee associated with the activity from the participant’s plan.

A short notice cancellation: The participant has given less than seven (7) clear days’ notice for a support.

STOPPING OR PAUSING MY THERAPY SUPPORTS:

You have the right to stop regular therapy supports at any time.

CONSENT TO OBTAIN / RELEASE INFORMATION:

Collaborative care is most beneficial in achieving therapeutic support outcomes. We will therefore often liaise with relevant therapists. By signing this agreement, you consent to us obtaining and sharing information with:

CONSENT TO DRIVE PARTICIPANT:
By signing this agreement, you consent to staff driving participants in staff vehicles when required. Staff and/or director of Cairns Allied Health Assistant and Enso Support Assistants are not held liable or take responsibility for injury/harm caused to the participant in the event of a vehicle incident/accident.

BREAKAGES:
By signing this agreement, you agree that Participant, Parents/ guardians agree to take responsibility for costs associated with damage to property or therapeutic support equipment incurred by the participant OR associated persons.

LIABILITY FOR INJURY OR HARM:
By signing this agreement, you agree that staff and/or director of Cairns Allied Health Assistant, and ENSO support services will not be held liable or take responsibility for injury/harm caused to the participant while receiving our support services.

MEDICAL ACKNOWLEDGEMENT:
By signing this agreement, you agree that the Staff and/or director of Cairns Allied Health Assistant and Enso Support Assistants will not be held liable or take responsibility for a participant's medical emergency during an accident and/or medical episode whilst under our care.

By signing this service agreement, you acknowledge that you are authorised to sign on behalf of the participant, that the information you have provided is true and correct, and that you agree to the terms of this agreement. You are not able to hold staff and/ or director of Cairns Allied Health Assistant and Enso Support Assistants responsible in the event of injury or harm to the participant. You agree to take responsibility for relaying important information including but not limited to the participant's health and medical conditions.
